Framework development of power enterprises' accident analysis system based on failure mode

Abstract: In order to ensure safe and efficient operation of electric power enterprises, according to causation theory and basic principle of safety system process, an accident analysis system based on failure mode was established. Data of management failure points were analyzed and corresponding personnel behaviors were revealed. The results show that failure in management process is caused jointly by human, organizational and management factors. Among them, human error and management failure are the main causes of accidents, accounting for more than 40% of them, while organization management failure accounts for more than 11%, and equipment for only 6%. Traditional analysis methods are mostly staying on the surface as they focus more on individual reasons, but give little attention to those of organization and system, and they are lack of in-depth analysis on management reasons. It is necessary to determine corresponding failure mode and provide solution strategies through analysis of accident causes so as to identify and control potential safety risks.
